Court Referral Process:
Step 1
Submit Court Entry to E.B. Evaluations, Inc. via email at info@ebevals.com or fax (740-777-6084)

Step 2
You will receive a call to confirm your Court Entry was received within 24 hours of submission
Step 3
E.B. Evaluations will contact the prosecuting attorney’s office and ask for Discovery for all cases listed on the Court Entry
A call will then be placed to the defense attorney asking for the Defendant’s information and additional records if applicable
Step 4
Once Discovery information has been received, efforts will be made to contact the defendant and schedule the evaluation as soon as possible
Non-Court Related Referral Process:
Step 1
Email info@ebevals.com with details regarding the type of evaluation you are requesting.
Step 2
Someone will respond to you within 24 hours asking you to complete a referral form to collect general information. Submit completed referral forms to info@ebevals.com or via fax (740-777-6084)
